"100% Worth It": How Hamilton Community Health Made Patient Outreach Pay for Itself (and then some…) in 90-Days

Hamilton Community Health Network has been caring for patients since 1982, serving the Flint, Michigan community through a growing network of practices. Their FQHC currently includes approximately 10 practices, with another location opening, 32 providers, and about 24,000 patients served each year.
As Hamilton’s Chief Operating Officer (and a Nurse herself), Cecilia Rutherford, MSN, BSN, RN oversees day-to-day operations, clinical coordination, and patient access across the network. With more than 20 years of nursing and leadership experience, she brings both an operational and clinical perspective to improving how Hamilton connects with patients and keeps care moving.

Communication That Feels Timely and Easy to Act On
Hamilton partnered with Vital Interaction to automate patient communication, reduce no-shows, and support more targeted outreach across the network.
With Vital Interaction, Hamilton was able to send personalized reminders through text, email, and phone calls. Messages included important details like provider name, location, address, and a click-to-call option, making it easier for patients to confirm, remember, and schedule appointments.
One example Cecilia shared was a patient who received a Sunday reminder before a Monday appointment. The patient had forgotten about the visit, called the call center, and was thankful for the reminder. For Hamilton, that kind of outreach helps patients plan ahead for transportation, childcare, and other needs that can affect whether they make it to their appointment.
Vital Interaction also helped Hamilton re-engage patients who had not been seen in two or more years. By setting outreach parameters, Hamilton could identify patients who were overdue for care and encourage them to schedule with their provider. Cecilia noted that patients who had not been in for years began coming back into the clinic because they received a timely reminder that Hamilton was still there and ready to care for them. As a nurse herself, she immediately recognized the impact of closing care gaps for these reengaged patients.
Reaching Patients Without Adding More Calls
One of Hamilton’s biggest operational concerns was the network’s no-show rate.
The team needed a better way to remind patients about upcoming visits, bring missed appointments back onto the schedule, and reconnect with patients who had not been seen in years. At the same time, Hamilton wanted to avoid placing more work on already busy staff and avoid hiring an additional call-center.
Before Vital Interaction, the alternative would have required a significant amount of manual outreach. Staff were spending time making calls, tracking down patients, and trying to manage appointment reminders manually.
Hamilton also wanted outreach to feel more personal. Generic EMR reminders did not provide the same level of detail or patient-friendly experience. Patients needed clear communication that included their provider name, appointment location, phone number, and an easy way to take action directly in-message.
From Appointment Reminders to Care Gap Outreach
Hamilton also used Vital Interaction to support targeted campaigns around important clinical priorities.
One of the most meaningful campaigns focused on patients who had not completed an A1C in the last year. Working with the medical director and Vital Interaction, the team created patient-friendly messaging about the importance of knowing their numbers and coming in for care. Cecilia also noted the value of similar outreach around hypertension, diabetes, and other care gaps that can impact long-term health outcomes.
Beyond reminders and reactivation, Hamilton used Vital Interaction to support operational updates. When opening their newest clinic location, Hamilton was able to notify patients of specific providers, and direct them to the correct building without relying on manual phone calls or staff stationed at the door. Vital Interaction helped send targeted messages ahead of the opening, allowing the transition to run smoothly with less manpower, and an already-booked schedule on Day 1.
Hamilton also has messaging prepared for unexpected closures, power outages, or urgent clinic updates. Instead of calling every patient manually as they had done in the past, the team can notify affected patients with one button press, and allow the call center to focus on rescheduling.
A Return That Goes Beyond the Numbers
Since implementing Vital Interaction, Hamilton has seen meaningful operational impact.
Cecilia shared that Hamilton’s network-wide no-show rate experienced approximately 15% reduction, with the rate continuing to decline. She also noted that the platform has made a clear difference in encounters and patient follow-through.
Hamilton has also seen growth from targeted outreach. For example, OBGYN patient volume increased after patients received messages letting them know that a highly requested provider was now offering OB services at Hamilton.
From an ROI perspective, Cecilia described the return as “100%” worth it. Their investment has paid for itself for the year through patients returning to the clinic and completing services with just these initial campaigns. Just as importantly, Vital Interaction has helped remove the heaviness of manual outreach from staff who were previously spending time making phone calls, tracking patients, and trying to bring people back into care.

Building on What’s Working
Hamilton is continuing to expand how it uses Vital Interaction for patient education, outreach, and access.
Next, their team plans to use VI campaigns to support Medicaid redetermination, helping patients understand what paperwork they need to complete and connecting them with available resources. Hamilton is also continuing to use targeted outreach to support OB growth, chronic care management, care gap closure, and operational updates across locations.
